AF Form 978 – AF 978 Form Printable, Fillable PDF – The purpose of AF Form 978 is A Supervisor’s Mishap Report (AF Form 978) is a document that is needed to help safety experts identify persons and obtain personal information that is required to complete mishap reports.
The Air Force (AF) of the United States of America (USA) issued the most recent version of the form on April 9, 2014, rendering all earlier versions outdated. A fillable version of the AF Form 978 is available for download below, or you may get it via the Air Force e-Publishing website.
Detailed information and filing standards may be found in Air Force Instruction (AFI) 91-204, Safety Investigation and Hazard Reporting, which can be viewed on the internet. These instructions give general direction for investigating and reporting any safety issues that occur under the United States Air Force’s jurisdiction. In addition, it applies to all levels of safety employees at all levels, as well as those who examine and report Air Force occurrences, as well as those who deal with such complaints. Instructions For Completing AF Form 978
The mishaps are categorized based on the overall direct cost of the accident as well as the severity of the injury or occupational sickness, and they are divided into the following categories:
- The occurrence of an unplanned event, or a succession of unplanned events, that results in property damage to the United States Department of Defense (DoD).
- Personnel suffering from occupational sickness
- Military soldiers that are injured, whether on or off duty,
- Injuries to civilian employees who are on the job;
- Infliction of harm to public or private property;
- injury or disease as a result of the Department of Defense’s activities.
In order to determine the severity of an injury or illness and to ensure proper reporting accountability within the Air Force, this form is used to identify the causes of illness or injury so that supervisors and functional managers can take appropriate action to eliminate or control unsafe and unhealthy conditions, to prepare statistical and historical reports as required by the Department of Defense, and to provide documentation for the cumulative summation of treatment reasons.
Any injuries ranging from mild to severe must be documented in the supervisor’s mishap report, regardless of whether they happened on or off the job. The paper is subsequently sent to the safety office for filing. The form is used to record a ground accident and forward it to the unit commander and Wing Safety for further investigation. The installation ground safety manager will analyze the form to decide whether or not it should be reported and to verify that the proper paperwork is in place. It is expected that the form will be completed and sent to the relevant safety office within five working days after the accident or when notified of the accident.
Mishaps are described in detail in AF Form 978, which includes both factual and analytical information. Information about the incident includes the following fields: full name, grade/rank, age, and sexual orientation; unit or office symbols; job titles; and the dates and times of the incident; when the incident occurred; duty status; scheduled work days; work shift hours; hours on duty prior to the incident; weather conditions; light conditions; number of days since the last deployment; number of days deployed in the previous 365 dayslocation of the incident site; description of the incident; witnesses and their names; disposition of the incident.
The section titled “Property Damage” contains the following information: a description of the property; a description of the vehicle; the type and date of training; a description of the property and vehicle damage; the cost of repair and materials; whether a seatbelt was worn; whether speeding was involved; and whether alcohol was involved. The names of these authorities, as well as their grades and positions, duty phone numbers, and signatures, are included in the section under “Supervisor, Unit Safety Representative, Commander, and Safety Office Review and Signature.”
The safety office fills up the following boxes: reportable (choose yes or no from the drop-down menu), the AF Safety Automated System number, the mishap class, the date the incident was reported to safety, the non-reportability reference, and the signature.
